Care program: Musculoskeletal (MSK)

What is the Musculoskeletal (MSK) care program?

Wellframe’s Musculoskeletal (MSK) care program offering includes 2 unique digital care programs:

  • Chronic Back Pain
  • Total Knee Arthroplasty

Both digital care programs span 30 days. The Chronic Back Pain care program is designed for any adult managing chronic back pain. To be enrolled in the Total Knee Arthroplasty digital care program, a member must also be enrolled in the Surgical Care Transitions Foundation program and have recently been discharged after a total knee arthroplasty.

How does Wellframe’s MSK care program empower care teams?

Digital care programs like the Musculoskeletal offerings can help members stay in contact with their care teams while they manage their pain and recovery. Plus, care managers can add members to more than one care program to meet their specific needs—such as with the Total Knee Arthroplasty and Surgical Care Transitions programs.

Digital care programs can make it easier for members to stay in contact with their care teams and build trusting relationships. Members are also better able to take an active role in their health.

Surveys and assessments surface key insights

As part of the MSK program, care managers can send members digital surveys and assessments that flag potential care gaps or barriers to healthcare access. Care managers can use validated screening tools such as:

  • PROMIS Pain Interference
  • PROMIS CAT Depression
  • PROMIS Physical Function

Care teams receive dashboard alerts to flag high-risk members and make it easier for clinicians to prioritize outreach.

What types of information do Musculoskeletal care program participants receive?

All the digital care programs in the MSK programs include educational articles, digital surveys, assessments, and customizable encouragements. Program participants have a daily checklist with relevant health and wellness tasks to complete.

Checklists encourage members to take action to improve their health and wellness. Actions can include medication reminders, physical activity tracking, logging water intake, and reading articles about their diagnosis and health benefits. A few of the articles recommended as part of the CKD program offerings are:

  • Staying Active
  • Eating to Recover

Digital care programs like those in Wellframe’s Musculoskeletal care program offering can help care teams build trust with members and uncover the information they need to improve outreach and care outcomes.
