What is a digital care program?
Wellframe’s digital care programs are designed to support the full range of clinical and non-clinical patient population needs, supporting all members as they move along their healthcare journeys. Members’ health needs are fluid, and will require varying levels of support and resources over time. Programs are built in collaboration with clinicians to help patients better manage acute and chronic conditions. Currently, Wellframe offers more than 70 digital care programs across 10 clinical areas. They are designed to be holistic and accessible, encompassing content that addresses medical, behavioral, and social conditions that can impact a member’s healthcare journey and outcomes. The program is delivered via the mobile member facing app.
