Engaging members with chronic conditions like diabetes and hypertension can be challenging. Untreated, these conditions can lead to complications like heart attack and stroke. Remote patient monitoring may be helpful for giving health plans insight into member progress at home, but tracking biometrics is just the first step.
To truly affect outcomes and close gaps in care, plans need a window into their members’ health needs at home. In this webinar, two healthcare leaders will discuss new data from Wellframe’s Member Impact Reports, which offer a view into engagement trends plus insights that can improve chronic condition management. Attendees will learn:
- Key strategies to reduce blood sugar and blood pressure among members with diabetes and hypertension
- The correlation between digital health experience and outcomes
- Considerations for evaluating a biometrics solution
