New mobile resources help health plans deliver vaccine information and more COVID-19 support, at scale

As the coronavirus pandemic continues across the United States, it’s crucial for people to have access to trustworthy and easy-to-understand information in order to stay safe and healthy. This need is especially high among populations that are more vulnerable to COVID-19, who may not be able to readily access accurate information about the new COVID-19 vaccines. To help our health plan customers continue to support and engage their members through the pandemic and vaccine rollout, Wellframe created three care programs:

COVID-19 Vaccine Education and Tracking

We have made our COVID-19 Vaccine Education and Tracking Program available to all customers. In the United States today, more Americans have received at least one dose of the vaccine than have tested positive for the virus since the pandemic began. But even with this promising progress, we won’t reach herd immunity until we achieve a critical mass of vaccinations. That’s why it’s critical to build trust, help people understand their risk, and connect them to the right resources when they become eligible. 

Wellframe’s COVID-19 Vaccine Education and Tracking Program helps health insurance plans provide members with accurate information about how the vaccine works, where they can get it, and what they can expect when they do receive the vaccine. The COVID-19 Vaccine Education and Tracking Program also connects members with their care manager to help build trust and confidence among vulnerable populations who may mistrust the healthcare system. This program consists of educational articles as well as a survey that can uncover members’ barriers to getting the vaccine and what concerns they may have about it.

When a member is eligible for the vaccine, the program can collect and track member-recorded dates between the first and second vaccine doses and provide reminders to ensure members get their second dose.

COVID-19 Assessment and Resources

Now that we’re a year into this crisis, we know many have eased their precautions as “COVID-fatigue” sets in. It’s also critical to remember the continued socioeconomic effects of the pandemic. This mobile care program gives customers a chance to check in with their members, provide a refresher on resources and protocols, and evaluate the need for support or referrals. The program sends an assessment to members that addresses screenings for clinical, social, and economic needs, as well as knowledge checks to promote adherence to COVID-19 guidelines and recommendations. Members will also receive an article entitled “Fast Facts: Coronavirus” that provides critical reminders about COVID-19 and links out to helpful resources related to navigating the pandemic.

Social Isolation

Quarantine and lockdowns have made it difficult for people to see their friends and loved ones. According to Health Affairs, people who are at a disproportionate risk of getting COVID-19—including older adults, people of color, people with low income, and people living in congregate housing—are also at high risk for social isolation.

Because of the risk of social isolation, we have made our Social Isolation Program immediately available to all customers. This program is designed to evaluate members’ social support and mental health, help members set small goals to start building connections, and escalate care for members who may need additional support. A brief module provides resources and supportive education on topics such establishing a routine, developing a communication plan, and tips for managing stress and being more active. The program also includes a follow-up check-in with members to evaluate their progress and determine if additional interventions are needed.
