Exploring a member’s health status is integral to fostering a partnership between the member and their health plan. Intake assessments are used by many of our customers to help to gather relevant member insights needed to curate personalized care plans and address the member’s most pressing health needs.
The traditional method for delivering these intake surveys has typically been telephonic. While familiar, this approach can lead to avoidable complexity, repetition, and excessive duration, with an unintended net result of suboptimal experience for both members and staff.
The Wellframe digital intake assessment offers a chance to more efficiently triage and connect members to care teams, services, and resources. The assessment will be paired with a general health education program that will function to deliver immediate value to members and allow them to further explore the merit of engagement with their care team. This streamlined experience can also help to reduce the burden of telephonic outreach and allow health plan staff to perform at the top of their license.
As we pilot this digital model in the upcoming quarter, we plan to leverage key learnings to iterate and improve the offering. This pilot allows us to test the deployment of a standardized assessment, designed based on learned digital best practices, that may be used by health plans at large to more efficiently scale the number of members they are able to support. We look forward to continued collaboration among our customers as we further expand the depth of members that we can support through the Wellframe platform.
